VON Canada is seeking a Directing Registered Nurse (DRN) to provide remote clinical leadership for a diverse caseload. You’ll manage pediatric, palliative, and acute medically complex clients, primarily with COPD or CHF, by directing Personal Support Workers using an innovative care delivery model and electronic documentation.
What You'll Do
- Provide remote assessment and management of care services for clients in their homes.
- Give clinical direction and assign tasks within the unregulated care provider’s practice statement.
- Maintain accurate client clinical records in both digital and paper formats.
- Manage patient assignments within the virtual ward.
- Work collaboratively with all team members to ensure patient care needs are met.
- Utilize effective communication in daily huddles and when responding to emails and phone calls.
- Communicate with outside service provider agencies and Case Managers as appropriate.
- Participate as a valued member of the multi-disciplinary health care team.
- Provide clinical leadership, direction, and mentorship within the team.
- Participate in and help facilitate a collaborative, team-based environment.
- Establish and maintain relationships with local health care providers.
- Participate in CNO Reflective Practice.
- Uphold VON standards and eHomecare-specific processes.
- Participate in internal projects and ongoing improvement initiatives.

What We're Looking For
- Registration in good standing with the College of Nurses of Ontario.
- A CNA Certificate in Palliative Care OR completion of both ‘Fundamentals of Palliative Care’ and ‘Comprehensive Advanced Palliative Care Education (CAPCE)’.
- For Pediatric DRNs: extensive hands-on experience in Pediatric Care provision.
- Recent experience with palliative, pediatric, and/or acute medical care for COPD and CHF patients.
- Demonstrated advanced assessment skills.
- Experience with digital and electronic health tools, including EMR and mobile technology.
- Proficient nursing skills with the ability to read and interpret electronic technical information related to client outcomes.
- Critical thinking, problem-solving, and multi-tasking skills.
- Ability to work in a multidisciplinary virtual team and independently.
- High-speed internet, computer skills, and a private home office space.
- Eligibility to successfully obtain a Government of Canada security screening certificate.
- Valid CPR C / First Aid certification.
- Access to a personal vehicle for business use, a current driver’s license, and proof of vehicle insurance.
- A current, original copy of a satisfactory Criminal Records Check upon start of employment.
- Must be able to wear Personal Protective Equipment (PPE).
Nice to Have
- Experience in gerontology and/or medical-surgical care.
- Experience working in a community-based health care system.
- Demonstrated competencies in observation, independent assessment, health education and promotion, and familiarity with COPD/CHF clinical pathways.
- Ability to speak French is an asset in French Designated areas.
