DaVita is seeking a RN Case Manager who is passionate about revolutionizing kidney care. In this full-time hybrid role based in Winchester, VA, you will provide critical support to patients with complex CKD/ESKD, coordinating quality, consistent, and cost-effective care by navigating a challenging healthcare system.
What You'll Do
- Support complex patients by assisting them in navigating a challenging healthcare system.
- Identify medical, social, emotional, and financial needs of patients through medical record review and consultation.
- Implement appropriate interventions based on patient needs.
- Work autonomously and in collaboration with all members of the healthcare team.
- Coordinate and facilitate quality, consistent, cost-effective care.
- Quickly build empathetic relationships with patients and families.
- Coordinate care including care transitions, management of complex/at-risk patients, managing ongoing needs, and establishing a treatment plan in partnership with the care team.
- Identify and address social determinants of health including medical, emotional, education, community, and financial concerns.
- Coordinate for streamlined and effective transition of care when needed.
- Assess patients and work to identify unreported medical conditions or changes that may lead to adverse outcomes.
What We're Looking For
- Current RN License
- Minimum of 5 years RN experience required with healthcare performance coaching
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
- Current driver’s license and willingness to travel within an assigned territory
- Intermediate computer skills and proficiency in MS Word, Excel & Outlook
- Private Home office with internet connectivity at a minimum of 1MB upload and 1MB download speed
- Must live locally in the Winchester, VA region
Nice to Have
- BSN preferred
- 2+ years of Case Management or Chronic Care Management
- 1+ years of utilization management experience
