CareHarmony was looking for a Remote - Licensed Practical Nurse (LPN) - Illinois Licensure

CareHarmony is looking for a Remote Licensed Practical Nurse (LPN) with an active Illinois license to join our team as a Care Coordinator. In this role, you will work comprehensively with providers to deliver value-based care management initiatives, directly impacting patient outcomes.

What You'll Do

  • Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
  • Identify and coordinate community resources that would benefit patient care.
  • Provide patient education and improve health literacy on the management of chronic conditions.
  • Perform medication management, including identifying potential concerns, reconciliation, adherence, and coordinating refills.
  • Ensure timely delivery of services such as Home Health, DME, and Home Infusion.
  • Resolve patient questions and create an open dialogue to understand their needs.
  • Assist in managing referrals and appointment scheduling.

What We're Looking For

  • Active Illinois LPN license.
  • At least 3 years of direct patient-facing work experience.
  • Technical aptitude with the Microsoft Office Suite.
  • Excellent written and verbal communication skills.
  • Must have active high-speed Wi-Fi.
  • Must have a home office or HIPAA-compliant workspace.

Nice to Have

  • Experience with Epic electronic health records.
  • Bilingual communication skills.
  • Additional state LPN licensures.
