CareHarmony is hiring a Population Health RN to deliver general nursing care for Evernorth’s total population in Northwest Georgia. You will use data-driven tools to perform comprehensive health risk assessments, provide health education, navigate benefits, and coordinate care, all aimed at improving health outcomes across defined populations.
What You'll Do
- Perform comprehensive health risk assessments and patient education.
- Follow established nursing protocols and procedures.
- Monitor medication compliance and provide related patient education.
- Educate patients on chronic conditions, focusing on abnormal biometric values and behavioral lifestyle modifications.
- Utilize motivational interviewing to promote sustainable changes in diet, exercise, and stress management.
- Identify customer health education needs through targeted assessments.
- Identify and help overcome psychosocial barriers to care.
- Collaborate with patients to establish health improvement plans, set evidence-based goals, and support achievement.
- Track and monitor screening compliance.
- Facilitate and promote preventive health screenings, including cancer screenings.
- Use frequent check-ins to reinforce understanding and adherence to care plans.
- Facilitate communication and coordination with internal and external providers, including primary care and specialists.
- Coordinate with community providers and resources to address patient needs and care gaps.
- Offer concierge-style coordination of appointments, referrals, and follow-up care.
- Reinforce provider’s care plans and ensure patient adherence.
- Advocate for patient needs within the care team and with matrix partners or systems.
- Go above and beyond to troubleshoot issues.
- Follow up with patients regarding after-hours care recommendations.
- Make referrals to clinical programs and services.
- Assist patients in navigating their benefits and accessing necessary resources.
- Advocate for patients to ensure they receive timely and appropriate care.
- Provide support for health-related site events like open enrollment, wellness committees, flu shot events, and health fairs.
- Serve as a trusted point of contact for patients and families, available for questions and guidance.
- Participate in new hire orientation or wellness campaigns at onsite locations.
- Work from reports and dashboards to proactively engage patients in care management.
- Chart within the Electronic Health Record (Epic) per current policies.
What We're Looking For
- RN with a current unrestricted compact state licensure in Georgia.
- At least three or more years of clinical practice experience.
- Current BLS/CPR/AED Certification.
- Excellent computer and interpersonal skills.
- Ability to work in a team environment with flexibility and enthusiasm for learning new skills.
- High energy level and a passion for health improvement.
- Positive role model in demonstrating healthy behaviors.
- Self-direction and the ability to succeed in an independent role.
- Must reside in Northwest Georgia.
Nice to Have
- Bachelor's degree in a health-related field.
- Five or more years of clinical practice experience.
- Demonstrated commitment to ongoing education.
