Allegheny Health Network is hiring a Patient Transition Coordinator for the Pittsburgh region. In this role, you will facilitate and monitor the referral workflow from the acute care hospital or outpatient setting to home. You will work directly with the Transitional Care Coordinator (TCC) to coordinate patients' post-acute care needs.

What You'll Do

• Secure services for post-acute care identified by the clinical care team via established workflows.
• Partner with the clinical team to obtain necessary supportive clinical documentation for compliance, regulatory, or insurance/billing requirements.
• Assure delivery of equipment via internal hospital or external partners following established workflows.
• Escalate issues and barriers to clinical partners when alternative planning is necessary due to staffing capacity, geography, insurance, or clinical complexity.
• Engage patients and/or their families to obtain choices for post-acute care as indicated by policy and document according to workflow.
• Build and maintain relationships with post-acute care and hospital/network partners.
• Respond to consumer inquiries regarding post-acute services.
• Participate in miscellaneous duties and rotational work on-site at network hospitals, including process improvement efforts, huddles, and special projects.

What We're Looking For

• High school diploma or GED.
• A clinical or medical background.
• Data entry experience.
• 1 year of experience working with Electronic Medical Records (EMR) systems.

Nice to Have

• An Associate’s Degree.
• 1-3 years of experience in the medical profession.
• 1 year of Customer Service experience.

Work Mode

This position operates on a hybrid schedule and is based in the Pittsburgh area.
