21st Century Home Health Services Inc. is hiring a full-time Physical Therapist Case Manager to provide skilled home health services. In this field-based role, you will be responsible for direct patient care and comprehensive case management for clients in their homes.
What You'll Do
- Provide direct physical therapy services to patients in their homes
- Manage a caseload of home health patients, coordinating their care plans
- Conduct patient assessments and develop individualized treatment goals
- Document patient progress, outcomes, and care plans accurately and timely
- Collaborate with other healthcare professionals involved in patient care
What We're Looking For
- Current California Physical Therapist license in good standing
- Graduate of an accredited physical therapy program
- Ability to travel to and work in patient homes within the service area
- Strong clinical assessment and treatment planning skills
- Excellent communication and documentation abilities
Work Mode
This is a full-time, on-site position requiring daily travel to and work within patient homes. You must be based in or able to commute to the San Francisco, California area.
