Support individuals facing complex behavioral and medical challenges in a role centered on compassion, connection, and community. As a Community Guide - Behavioral Health, you’ll work directly with members managing serious mental illness, substance use disorders, and chronic health conditions, building trust and guiding them toward meaningful care and resources.
What You’ll Do
- Establish and maintain a caseload of 60–100 members through proactive outreach, building relationships rooted in empathy and respect.
- Conduct assessments to understand each member’s unique needs, including social determinants of health, and contribute to personalized care planning.
- Collaborate with clinicians and care teams by joining visits, supporting care coordination, and following up on social and behavioral health needs.
- Connect members to essential services—including behavioral health, primary care, and community programs—and support appointment scheduling and attendance.
- Use in-person visits, phone, text, and email to engage members in ways that align with their preferences and circumstances.
- Document all interactions thoroughly using digital tools, including updating care plans in the Reema App and maintaining accurate records in electronic health systems.
- Transport members to appointments when necessary and support access to services that promote independence.
- Work independently while contributing to a collaborative, multidisciplinary team in a fast-paced, evolving environment.
- Adapt to change with resilience and maintain professional boundaries, confidentiality, and compliance with HIPAA standards.
What We’re Looking For
- Bachelor’s degree in Social Work, Psychology, Human Services, Health Sciences, or a related field—equivalent experience considered.
- At least three years of direct experience serving populations with serious mental health or substance use challenges.
- Strong interpersonal skills with a proven ability to initiate and sustain supportive relationships.
- Proficiency with digital tools, including Microsoft Office, video conferencing, and electronic health records.
- Valid driver’s license, reliable transportation, and ability to pass a background check and drug screening.
- Ability to lift up to 25 pounds and travel locally for home and community visits.
