Mass General Brigham is hiring a Per Diem RN Case Manager to coordinate services and treatments for ambulatory oncology patients throughout their care journey. This fully remote position collaborates closely with a care team including physicians, APPs, practice RNs, pharmacies, vendors, patients, and families.
What You'll Do
- Maintain accurate, detailed reports and records.
- Administer medications and monitor patients for reactions or side effects.
- Record patients' medical information and vital signs.
- Monitor, record, and report symptoms or changes in patients' conditions.
- Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
- Modify patient treatment plans as indicated by patients' responses and conditions.
- Coordinate home-based services, including VNA, hydration, oxygen, and durable medical equipment.
- Obtain prior authorizations for medications and treatments.
- Document all coordination in the electronic health record.
- Collaborate with physicians, APPs, ambulatory practice RNs, pharmacies, vendors, patients, and families.
What We're Looking For
- Associate's Degree in Nursing
- Registered Nurse [RN - State License]
- 0-1 years of clinical nursing experience
- Foundational knowledge in oncology care
- Willingness and ability to learn intricacies of insurance regulations, including state and federal Medicare/Medicaid policies
- Familiarity with principles and skills for practical nursing to provide patient care
- Knowledge of care required by respective age groups
- Ability to maintain confidentiality and secure sensitive information
- Knowledge of medical terminology
- Excellent verbal and communication skills
- Ability to accurately screen and triage acute patients
