Sentara Health is hiring a Palliative APP (Nurse Practitioner or Physician Assistant) to join the health care team at Sentara Leigh Hospital. You will provide clinical care, support, and education for the palliative patient population, advancing our mission to improve health every day.

What You'll Do

Perform palliative care consultations and follow-up visits for hospitalized patients with acute palliative care needs, including symptom management and complex medical decision-making.
Facilitate family meetings related to current medical decision-making and advance care planning.
Assist with developing appropriate discharge plans with respect to established goals of care, including close collaboration with Hospice to ensure access and continuity of care.
Actively participate as a member of an interdisciplinary team to ensure palliative care addresses pain and symptom management, psychosocial/spiritual support, and caregiver support.
