About the Role

CVS Health is hiring a Nurse Practitioner, Advanced Practice Provider to deliver effective and equitable value-based primary care to adults on Medicare. Your purpose is to keep patients happy, healthy, and out of the hospital through independent patient visits and collaboration with physicians.

What You'll Do

  • Provide exceptional, longitudinal primary care and build meaningful patient relationships.
  • Conduct office visits for routine and acute health issues.
  • Administer Annual Wellness Visits and Health Risk Assessments with holistic, accurate documentation.
  • Coordinate care with other providers, specialists, and agencies.
  • Lead population health initiatives, ensuring patients receive evidence-based screenings and preventive care.
  • Assist the care team with phone triage and patient outreach.
  • Educate patients on their health conditions, care plans, and treatments.
  • Participate in promotional activities and conduct home visits as needed.

What We're Looking For

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification as a Family, Adult-Gerontology Primary Care, Adult, or Gerontological Nurse Practitioner.
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA license.
  • US Work Authorization.

Nice to Have

  • Experience in primary care specialties: internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments.
  • Passion for teamwork and cross-functional collaboration.
  • Desire to be part of an innovative model focused on empirically-guided population health.
  • Bilingual proficiency in applicable areas.

Team & Environment

You will practice on a collaborative care team composed of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. You will report to the Center Medical Director.

  • Salary Range: $95,738.00 - $206,206.00
  • Quarterly bonus based on quality metrics.
