Humana is hiring a NICU Care Manager, Telephonic Nurse to join our caring community. In this role, you will assess and evaluate members' needs telephonically to achieve and maintain optimal wellness by guiding members and families toward appropriate resources. You will provide dedicated, compassionate support, specifically following our tiniest members from the beginning of their NICU stays until 30 days post-discharge.
What You'll Do
- Assess and evaluate members' needs and requirements in a dedicated telephonic environment.
- Guide members and families toward and facilitate interaction with resources appropriate for their care and wellbeing.
- Employ a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.
- Follow NICU members from the beginning of their stays until 30 days post-discharge.
- Identify and utilize appropriate healthcare resources and provide guidance along the healthcare continuum.
- Identify and resolve barriers that hinder effective care.
- Ensure patients are progressing towards desired outcomes by continuously monitoring care through assessments and evaluations.
- May create comprehensive member care plans.
- Make decisions regarding your own work methods, occasionally in ambiguous situations.
- Work from a dedicated, interruption-free space to protect member PHI and comply with HIPAA regulations.
What We're Looking For
- Active, unrestricted license as a Registered Nurse (RN) in your state of residence with no disciplinary actions.
- Active Compact RN license with the ability to apply for additional licensure without restrictions.
- 2+ years of case management experience.
- Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook.
- Ability to work independently under general instructions and collaborate effectively as a team player.
- Strong organizational skills and proven experience multi-tasking.
Nice to Have
- BSN or Bachelor’s degree in a related field.
- Registered Nurse (RN) with 3+ years of NICU Level 3 clinical experience.
- Experience with discharge planning and patient education for NICU, pediatrics, and adult acute care.
- Managed care experience.
- Certified Case Manager (CCM) credential.
- Bilingual proficiency, preferably in Spanish.

- Salary Range: $71,100 - $97,800 per year.
