What You'll Do
Provide direct nursing care to patients receiving acute treatment in their homes, functioning as part of an integrated care team. You'll work under the guidance of a registered nurse or physician, contributing to patient assessments, care planning, and ongoing clinical support. Your responsibilities include implementing nursing interventions, collecting patient data, and ensuring accurate documentation throughout the care cycle.
Support admissions and discharges, assist with remote monitoring technologies, and help coordinate logistics essential to home-based care delivery. You’ll collaborate with interdisciplinary professionals to maintain continuity of care and contribute to the continuous improvement of care processes.
Requirements
- Must have at least two years of recent experience in acute care, ICU, or emergency department settings
- Hold a valid New York State driver’s license
- Have reliable transportation for home visit travel
Preferred Qualifications
- Five or more years of diverse clinical experience
Benefits
Full-time roles are available with primary 12-hour day shifts, though evening, night, and 8-hour shifts may be required based on unit needs. Weekend and holiday coverage is expected per policy. This role offers the chance to be part of an emerging model that brings hospital-level care into patient homes, expanding the scope of acute care nursing.
Work Mode
This is an onsite position with assignments based out of the program’s central location. Schedules include rotating shifts across days, evenings, nights, weekends, and holidays in accordance with unit requirements.
