What You'll Do
Deliver personalized, high-quality care to patients ranging from infants to older adults at a well-established clinic in Rockford. You’ll work collaboratively with an experienced physician, contributing to a supportive and integrated practice environment. Your role includes managing your own patient panel, participating in routine and preventive care, and helping strengthen continuity within the primary care team. The schedule follows a consistent Monday through Friday format from 8 a.m. to 5 p.m., with one Saturday shift per month.
Requirements
You must be a graduate of an accredited nursing program and hold a master’s or doctoral degree in nursing from a recognized institution. Eligibility to practice as a Nurse Practitioner in Illinois or Wisconsin is required, along with active Illinois RN and APRN licenses and a current IL DEA registration. Experience in primary care nursing or as a Nurse Practitioner is strongly preferred.

Work Environment
This is an onsite position located in Rockford, IL, with a predictable weekly schedule. The practice uses EPIC for its electronic health records, ensuring streamlined coordination and documentation.
