Corewell Health is hiring a Community Health Worker Peer to Peer Coach to provide direct peer support and health education to individuals and families. In this role, you will use your personal experience to build trust and model healthy behaviors, contributing to our mission of improving community wellness.
What You'll Do
- Provide peer support and role modeling to individuals and their families during personal interactions, including home visits and telephone calls.
- Instruct individuals and their families about wellness, chronic disease prevention, and self-management.
- Assist clients and their families with accessing appropriate community resources.
- Assist with the coordination of group support programs and provide ongoing contact.
What We're Looking For
- High School Diploma or equivalent.
- Successful completion of the paraprofessional CHW Training Program (Completion required within 180 days from hire date).
- LIC-Driver's License from the State of Michigan.
- CRT-Auto Insurance.
Nice to Have
- Associate's Degree or equivalent.
- 3 years of relevant work experience.
- Knowledge and use of community resources; social, health and public agencies and services.
- Cultural competency and shared-life experiences with the client population.
