CVS Health is looking for a Case Manager RN to join our team. In this 100% remote role, you will play a key part in our mission to transform health care by caring for people where, when and how they choose. You will use a collaborative process of assessment, planning, facilitation, and advocacy to meet members' health needs through effective communication and resource management, promoting quality and cost-effective outcomes.
What You'll Do
- Apply data-driven methods to identify members and create individualized case management programs.
- Conduct comprehensive clinical assessments to evaluate needs.
- Develop flexible care approaches based on member needs, benefit plans, or external services.
- Advocate for patients to the full extent of their existing health care coverage.
- Promote quality, cost-effective outcomes and suggest improvements for program efficiency.
- Identify and escalate quality of care issues through established channels.
- Provide an expected very high level of customer service.
- Utilize assessment techniques to determine members' health literacy, technology capabilities, and readiness to change.
- Utilize influencing and motivational interviewing skills to maximize member engagement and promote lifestyle changes.
- Provide coaching, information, and support to empower members to make independent medical and lifestyle choices.
What We're Looking For
- Active current and unrestricted RN license in your state of residence.
- Willingness and ability to obtain additional state licenses upon hire (paid for by the company).
- 3+ years of acute care experience as an RN.
- Ability to work Monday through Friday, 8:00am through 4:30pm in your time zone, with an occasional late shift rotation per business needs (once per quarter).
- Associate Degree.
Nice to Have
- Compact RN License or Multi-state licensure.
- Certified Case Manager (CCM).
- Managed Care experience.
- Hospital experience in ICU, CCU, Step Down, ER, or OR.
- Telephonic case management or telephonic experience.
- Bachelor's degree.
