Spring Health is hiring a Care Navigator - Substance Use Disorder to play a pivotal role in eliminating barriers to mental health. In this full-time remote position, you will independently manage client care, apply expert clinical guidance, and coordinate comprehensive support for individuals with SUDs.
What You'll Do
- Conduct outbound calls and follow-ups with individuals interested in or referred for SUD treatment.
- Engage with clients using advanced clinical techniques to assess needs and provide information about treatment options.
- Independently coordinate appointments, referrals, and follow-up care, managing complex cases.
- Apply advanced psycho-education, motivational interviewing, and evidence-based practices to support client goals.
- Utilize in-depth knowledge of SUD, ASAM levels of care, and treatments like contingency management, MAT, and detox.
- Provide sophisticated in-the-moment support, crisis intervention, and safety planning for high-risk situations.
- Maintain detailed and accurate records within an EHR and other systems.
- Identify and resolve complex issues that may disrupt care.
- Collaborate effectively with multi-disciplinary teams including care support, provider operations, and benefits/billing.
- Actively participate in staff meetings, case consultations, and ongoing training.
- Adhere strictly to confidentiality, compliance, and ethical standards.
- Provide support and coverage to other SUD Clinical Navigators.
- Work hours: Mon-Fri 12:00pm-8:00pm Eastern Time.
What We're Looking For
- Mental Health Licensure required: LCSW, LMFT, or LPC.
- 2+ years of experience in substance use disorder (SUD) clinical care.
- Proficiency in motivational interviewing, triage, ASAM criteria, various levels of care, and best practices for SUD.
- Strong listening, written, and verbal communication skills.
- Ability to work effectively with diverse populations and a commitment to embracing diversity and inclusion.
- Motivated to provide a high-touch, supportive experience for clients.
- Comfortable using various computer platforms, learning new systems, and efficiently handling tech-related tasks.
- Capable of managing multiple tasks and switching contexts efficiently.
- Embraces new challenges and opportunities for development.
Nice to Have
- SUD licensure or certifications.
- Bilingual.
