Senior Care Manager - Registered Nurse
Role Overview
This position is responsible for delivering comprehensive care management services to members through clinical assessments, personalized planning, and ongoing support. As a licensed RN, you will play a central role in shaping care strategies that reflect each member’s unique needs, goals, and preferences.
Key Responsibilities
- Conduct initial screenings and in-depth assessments to evaluate member health status and support requirements
- Establish and maintain continuity of care for new enrollees by coordinating timely interventions
- Identify priority needs and tailor care approaches to individual member circumstances
- Create measurable goals within person-centered service plans that encourage independence and self-management
- Implement and regularly review care plans, supporting members in accessing appropriate accommodations
- Manage a defined caseload, using risk stratification to guide monitoring and reassessment schedules
- Support transitions between care settings and ensure follow-up protocols are followed
- Collaborate with primary care providers and specialists, incorporating feedback from members, caregivers, and clinical teams
- Provide mentorship and guidance to less experienced care managers to strengthen team capabilities
Qualifications
- Active Registered Nurse (RN) license
- Proven background in care management, clinical assessment, and care plan development
- Experience managing a diverse caseload and coordinating care transitions
- Strong interpersonal and communication skills for engaging with providers, members, and families
- Commitment to person-centered planning and ongoing care monitoring
Work Environment
This role operates in a flexible work environment with availability required between 10:30 AM and 7:00 PM. The position supports members in Florida, including the Miami area, with options for remote or location-based work as needed.
