Centene is looking for a Care Manager (RN) to develop, assess, and facilitate complex care management activities for members with primarily physical health needs. You will create personalized care plans and provide education for members and their families, transforming community health one person at a time.

What You'll Do

Evaluate member needs, barriers to care, social determinants of health, and member-identified priorities to recommend and/or facilitate the best outcome plan.
Develop ongoing care plans/service plans and collaborate with providers to identify specialists and community resources to address unmet needs.
Identify problems/barriers to care and provide appropriate care management interventions.
Coordinate between the member/family/caregivers and the care provider team to ensure adequate and appropriate person-centered care.
Provide ongoing follow-up and monitoring of member status, condition changes, and progress towards care plan goals; revise plans as necessary.
Provide resource support to members and care managers for local resources (e.g., employment, housing, independent living).
Facilitate care management and collaborate with providers to ensure timely access to needed care or services.
Perform telephonic, digital, home, and/or other site outreach to assess member needs and collaborate with resources.
Collect, document, and maintain all member information and care management activities for regulatory compliance.
Provide and/or facilitate education to members and families on disease processes, care gaps, provider instructions, care options, referrals, and benefits.
Provide feedback to leadership on opportunities to improve care and quality delivery in a cost-effective manner.

What We're Looking For

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
2 – 4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State Licensure
