Humana Inc. is looking for a Care Manager, Telephonic Nurse 2 to join our Patient Management Program. In this remote role, you will be a crucial part of our caring community, using your clinical expertise to evaluate members with specialized and complex health conditions and help them achieve optimal wellness.
What You'll Do
- Assess and evaluate member conditions with an emphasis on medication therapy through telephonic interactions.
- Develop and implement comprehensive care plans.
- Monitor patient progress through regular assessments.
- Utilize advanced telecommunication systems to provide medication education.
- Work within a call center environment with a set schedule of inbound and outbound tasks.
What We're Looking For
- A completed Bachelor of Science in Nursing (BSN) is required.
- 3 - 5 years of clinical acute care experience.
- Licensed Registered Nurse (RN) in the appropriate state with no disciplinary action.
- Must reside in OH, KY, FL, AZ, or TX.
- Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook.
- Managed care experience.
- A passion for contributing to an organization focused on continuously improving consumer experiences and strong schedule adherence.
Nice to Have
- Experience with care management and patient education for adult care.
- Auto dialer experience.
- Bilingual English/Spanish.
