Responsibilities
- Deliver high-quality preventive, acute, and chronic medical care through a fully virtual platform.
- Enhance the standard of remote patient care by conducting empathetic and impactful clinical interactions.
- Serve as the primary care provider for a designated group of patients.
- Coordinate patient care jointly with primary care providers and outside healthcare partners.
- Inform patients about suitable treatment options and personalized care strategies.
- Issue and renew prescriptions in accordance with clinical judgment.
- Oversee follow-up care for individuals enrolled in chronic disease management programs.
- Direct patients to specialty or emergency services when medically necessary.
- Engage in population health efforts focused on at-risk patients, including wellness visits, closing care gaps, care transitions, and risk documentation.
- Assist patients shortly after hospital discharge to reinforce treatment comprehension and prevent readmission.
- Partner with clinical colleagues to develop evidence-based care protocols and organizational policies.
- Conduct ongoing self-assessment to maintain compliance with clinical standards and recommended practices.
- Meet performance targets that emphasize efficient, high-quality care delivery using integrated digital tools.
