Responsibilities
- Deliver high-quality care for preventive, acute, and chronic conditions through a fully virtual platform
- Enhance the remote patient experience by conducting empathetic and impactful clinical interactions
- Serve as the primary care provider for an assigned group of patients
- Direct patients to specialty or emergency services when medically necessary
- Engage in population health efforts including annual wellness visits, closing care gaps, care transitions, and risk documentation
- Offer timely follow-up care after hospital discharge to reinforce treatment understanding and lower readmission rates
- Regularly assess clinical performance to maintain compliance with established guidelines and standards

Work Arrangement

- Required to complete three 12-hour shifts per week
- Scheduled shifts must include both Saturday and Sunday, plus one weekday
- Flexible start times, such as between 7:00 am and 9:00 am Pacific Time
- Work can be performed from home
- Care delivery is fully remote and conducted via video
- Around-the-clock support from Care Concierge, Clinical Operations, and technical teams
- Must hold an active medical license in Oregon
- Interstate Medical Licensure Compact (IMLC) license is a plus
- Prior hands-on experience with EPIC Electronic Medical Record system is mandatory
- Must have a clean background check and no history of medical malpractice claims
