Responsibilities
- Deliver high-quality preventive, acute, and chronic care through a fully virtual platform
- Enhance remote patient care by fostering empathetic and impactful clinical interactions
- Serve as the primary care provider for an assigned group of patients
- Direct patients to specialists or urgent care services when medically necessary
- Engage in population health efforts for attributed patients, including annual wellness visits, closing care gaps, care transitions, and risk documentation
- Assist patients shortly after hospital discharge to reinforce treatment comprehension and lower readmission rates
- Conduct ongoing self-assessment to maintain compliance with clinical standards and evidence-based practices

- Work remotely from home
- Work three 12-hour shifts per week
- Schedule includes weekends—Saturday and Sunday—plus one weekday

- Care delivered entirely through video-based virtual visits
