CenterWell Home Health, a Humana company, is looking for a Weekend Clinical Manager to ensure clients receive timely, coordinated, and compliant care. In this role, you will be responsible for the receipt, input, review, and completion of all weekend referrals and calls, directly supervising patient care.
What You'll Do
- Directly, or in coordination with Central Intake, manage Referral Intake to ensure assessment visits are scheduled and performed timely.
- Review orders on the action screen and approve or decline as appropriate.
- Enter and approve all orders for tracking and route them to Medical Records.
- Update client medication profiles and schedules as applicable.
- Enter detailed Non-Admit information into the system if no visit was made.
- Be available for all calls received during on-call hours, answer calls, and determine patient and family needs.
- Resolve issues in a timely manner and inform appropriate team members of actions taken.
- Reassess patient needs and update or revise the Plan of Care as necessary.
- Assure patient safety is maintained at all times.
- Verify physician orders and verbal orders.
- Accurately document patient observations, interventions, and evaluations on the day services are rendered.
- Develop Plans of Care on assigned cases for admissions or justification of continuous care as needed.
What We're Looking For
- Associate degree in Nursing (ADN).
- Active and unrestricted Registered Nurse (RN) license or Compact State RN license with authorization to practice in North Carolina.
- Proof of current CPR card and/or BLS certification.
- At least one (1) year of experience in an acute hospital or comparable setting.
- Self-motivated, assertive, and organized with excellent oral and written communication skills.
- Must read, write, and speak fluent English.
- Knowledge of medications and their correct administration.
- Ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
- Ability to communicate effectively with patients, their family members, and at all levels of the organization.
- Maintains current licensure, certifications, and meets mandatory continuing education requirements.
- Must have good and regular attendance.
- Maintains current and accurate records in accordance with policies, procedures, and Federal, State, and local guidelines.
- Protects the integrity of the organization, patients, and co-workers by maintaining confidentiality.
- Maintains a safe work environment by complying with all Safety/Risk/OSHA policies and procedures.
- Consistently complies with all company and regulatory policies, procedures, and guidelines.
Nice to Have
- Bachelor of Science in Nursing (BSN).
- Minimum of two (2) years of home care experience, with one (1) year of nursing supervisor or administrative experience.
- Knowledge of Medicare regulations.
