Responsibilities
- Deliver high-standard preventative, acute, and chronic medical care through a fully virtual environment.
- Enhance the virtual care experience by fostering empathetic and impactful patient interactions.
- Serve as the primary care provider for an assigned group of patients.
- Collaborate with primary care providers and external healthcare partners in shared patient management.
- Inform patients about suitable treatment options and personalized care strategies.
- Issue and renew prescriptions as clinically appropriate.
- Oversee follow-up care for individuals enrolled in chronic disease management programs.
- Direct patients to specialist services or urgent care when medically necessary.
- Engage in population health efforts targeting at-risk patients, including wellness visits and care gap resolution.
- Assist patients shortly after hospital discharge to reinforce treatment comprehension and prevent rehospitalization.
- Partner with clinical colleagues to develop and refine clinical protocols and guidelines.
- Conduct ongoing self-assessment to maintain compliance with clinical standards and evidence-based practices.
- Meet performance targets focused on delivering effective, high-quality care using integrated digital tools.
