Provides remote clinical support for a Washington State-based substance use disorder program serving individuals during prenatal and postpartum stages. Delivers individualized care through assessments, therapy, and care planning. Works collaboratively with families, providers, and interdisciplinary teams. Therapists must be licensed in Washington, Louisiana, or Florida and can work from any U.S. location.
Responsibilities
- Conducts personalized assessments and develops care plans aimed at the least restrictive and most effective level of care.
- Works with members, families, and healthcare providers to create individualized treatment plans.
- Provides individual counseling and facilitates group therapy sessions for adolescents and adults.
- Identifies and initiates referrals to social services, including financial, psychosocial, community, and state support programs.
- Advocates for members and families to ensure their needs and choices are represented within the care team.
- Uses established clinical criteria to evaluate members and determine appropriate levels of care.
- Maintains accurate documentation of assessments, care plans, and referrals.
- Meets performance goals and adheres to defined key performance indicators.
- Learns and applies the StrongWell care model with accountability for outcome-based results.
- Attends interdisciplinary team meetings and utilization management discussions to support safe care transitions.
- Ensures ethical and responsible use of organizational resources.
- Maintains consistent punctuality and attendance during scheduled work hours.
Requirements
- Must hold a current LCSW, LMFT, LMHC, or LPC license in Washington, Florida, or Louisiana.
Nice to Have
- Experience working with individuals with substance use disorders.
- Background in maternal health and substance abuse treatment.
- Experience providing therapy to both adults and adolescents.
- Familiarity with Medicare and Medicaid policies and procedures.
- Prior experience working remotely.
