Included Health is looking for a Virtual Primary Care Nurse Practitioner to join our clinical team. In this full-time role, you will provide comprehensive, high-quality primary care services to patients through a virtual platform.
What You'll Do
- Conduct virtual patient assessments, diagnoses, and treatment plans for a diverse patient panel.
- Manage chronic conditions, provide preventive care, and address acute health concerns remotely.
- Prescribe medications and order/interprets diagnostic tests as appropriate for virtual care.
- Collaborate with physicians, specialists, and other care team members to ensure coordinated patient care.
- Maintain accurate and timely electronic health record (EHR) documentation.
- Adhere to clinical best practices, regulatory standards, and Included Health protocols.
What We're Looking For
- Active and unrestricted Nurse Practitioner license in your state of residence.
- National certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).
- Minimum of 3+ years of experience practicing as a Primary Care Nurse Practitioner.
- Demonstrated proficiency with telehealth platforms and electronic health records.
- Excellent clinical judgment, communication skills, and a patient-centric approach.
- Ability to work independently in a remote setting while maintaining strong collaborative ties.
Work Mode
This is a full-time, fully remote position. You must be licensed and located within the United States.
