Vaughan Regional Medical Center / ScionHealth is seeking a Therapeutic Care Provider I to join our team. In this entry-level case management role, you will support inpatient, outpatient, and support services for adults, adolescents, and seniors experiencing psychiatric or addictive disorders. What You'll Do
- Conduct intake assessments and determine level of care.
- Assist in registration, insurance verification, and pre-certification.
- Facilitate referrals and link patients to appropriate services.
- Deliver basic therapeutic interventions and support services.
- Participate in interdisciplinary treatment planning and documentation.
- Travel to conduct assessments when needed.
- Integrate evidence-based practices into care delivery.
- Provide basic care support typically associated with a Nurse Tech.
What We're Looking For
- Bachelor’s degree in a related field.
- Basic Life Support (BLS) certification.
- Handle with Care Certification or CPI within 90 days of hire.
Nice to Have
- Master’s degree in Social Work, Psychology, or related field with a mental health specialization.
- Current professional license.
- Valid Driver’s License.
- Prior experience in mental health or social services.
