About the Role
The nurse will conduct telephonic assessments, develop care plans, and coordinate services for patients, focusing on improving health outcomes while maintaining compliance with regulatory standards.
Responsibilities
- Perform initial and ongoing patient assessments over the phone
- Develop individualized care management plans
- Monitor patient progress and adjust interventions as needed
- Collaborate with healthcare providers to coordinate treatment
- Ensure compliance with clinical guidelines and policies
- Document all case activities in the electronic health record
- Educate patients and families about care plans and resources
- Facilitate timely access to medical services
- Identify barriers to care and implement solutions
- Maintain patient confidentiality and adhere to HIPAA standards
- Conduct follow-up calls to evaluate treatment effectiveness
- Assist with transition of care between medical settings
- Support patients in managing chronic conditions
- Respond to urgent clinical concerns promptly
- Utilize critical thinking to assess patient needs
- Work within established scope of nursing practice
- Report significant changes in patient status to supervisors
- Participate in team meetings and case reviews
- Stay current with clinical best practices
- Maintain accurate and timely case notes
