Provide advanced nursing care to patients with chronic kidney disease through virtual consultations and remote monitoring. You’ll conduct thorough Comprehensive Health Evaluations, assess medical and psychosocial needs, and develop individualized care plans aimed at slowing disease progression and reducing hospitalizations.
What You'll Do
- Perform remote patient assessments and manage complex comorbid conditions using clinical judgment and prescriptive authority
- Interpret diagnostic data and adjust treatment plans in collaboration with nephrologists and care teams
- Address social determinants of health, including emotional and financial challenges affecting patient well-being
- Lead care coordination efforts to ensure alignment with medical directives and improve long-term outcomes
- Support population health goals by focusing on preventive care and risk reduction
Requirements
- Active, unrestricted Nurse Practitioner license in Washington state
- Minimum of two years of experience as a licensed NP
- DEA certification or ability to obtain within 90 days
- Current CPR/BLS credential
- Valid driver’s license and personal auto insurance
- Proven ability to work independently with strong clinical decision-making skills
- Intermediate proficiency with MS Word, Excel, and Outlook
- Reliable home internet and dedicated workspace required
Preferred Qualifications
- Five or more years as a Family NP, Primary Care NP, or in Internal Medicine, Nephrology, Cardiology, or Endocrinology
- Experience in value-based care models, Medicare Advantage, or Health Risk Assessments
- Familiarity with population health strategies that reduce hospital utilization
- Background using Cerner or comparable electronic health record systems
