Graham Healthcare Group is hiring a Sr. Director of Clinical Practice to identify, create, and implement clinical practice standards for in-home care management. You will establish the clinical assessment standards and care protocols necessary to deliver high-value clinical and financial outcomes across our home health regions.
What You'll Do
- Establish in-home clinical assessment and practice standards that align with organizational procedures and policies.
- Establish protocols and standards for the in-home disciplinary care team to improve patient experience and mitigate readmission.
- Use evidence to train and implement care standards in each home health region to drive favorable clinical and financial outcomes.
- Work closely with the SVP-HH to develop reliable reporting on organizational metrics, including creating KPIs, clinical audit tools, and productivity measures.
- Analyze in-field work and its impact on claims data.
- Prepare and conduct clinical case reviews to mitigate hospitalization, ER, and observation unit use.
What We're Looking For
- A minimum of 5 years in progressive leadership and management roles in healthcare administration within a multi-site company.
- Substantial experience establishing standards and implementing programs and systems for clinical care and service.
- Demonstrated success at the VP/Director level.
- Ability to provide leadership and vision, develop partnerships, and influence all levels of an organization.
Team & Environment
You will work closely with the SVP-HH.
