Responsibilities
- Assess clients to identify individual problem areas necessitating treatment
- Complete assessments
- Coordinate interdisciplinary team meetings
- Coordinate discharge planning efforts among the multidisciplinary treatment team
- Coordinate the overall treatment plan implementation for the client on the unit
- Work closely with nursing staff to ensure continuity of treatment efforts
