Role Overview
This position leads a critical function in patient safety by managing the full lifecycle of clinical investigations. The investigator evaluates incoming safety reports from various sources, applies clinical expertise to assess risk, and ensures timely, accurate resolution. The role emphasizes systemic learning, using data and case patterns to inform long-term improvements in operations, policy, and technology.
Key Responsibilities
- Review and respond to patient safety complaints from multiple channels, including direct submissions and call center reports
- Perform urgent clinical triage, often within 24 hours, to determine risk level using established clinical and operational criteria
- Classify cases as high, moderate, or low risk and identify related or duplicate reports to ensure cohesive case management
- Lead investigations from initiation to closure, maintaining ownership and accountability throughout
- Analyze clinical narratives and documentation to determine root causes and distinguish isolated incidents from systemic issues
- Follow defined escalation protocols, including notifying leadership and referring cases to oversight bodies when necessary
- Document findings, decisions, and rationale clearly in the case management system
- Refine triage guidelines, escalation thresholds, and closure standards based on operational experience
- Support migration of legacy cases into modern systems and help design sustainable close-out strategies
- Identify trends and recurring risks through post-case analysis and contribute to feedback loops that inform organizational learning
- Translate case-level insights into actionable recommendations for policy, training, and communication
- Support leadership reporting and transparency initiatives to strengthen public confidence in patient safety systems
- Collaborate with technology teams to shape case management platforms that reflect clinical workflows
- Ensure accurate, timely documentation to support dashboards, reporting, and case tracking
- Maintain compliance with HIPAA, PHI/PII handling, and federal security standards
Required Qualifications
- Registered Nurse (RN) license required
- Minimum of 10 years of progressive clinical experience in high-acuity settings such as acute care, critical care, perioperative services, or transplant
- Proven ability to apply independent clinical judgment in complex, high-risk situations
- Experience in patient safety investigations, quality improvement, risk management, or compliance
- Strong capability in rapid clinical risk assessment across diverse scenarios
- Skill in balancing standardized processes with individual case nuances
- Familiarity with EHRs and case management systems
- Ability to adopt and support implementation of new technology platforms
- Experience using dashboards, reports, and structured data to inform decision-making
Preferred Qualifications
- Advanced clinical leadership experience
- Background in organ donation, transplant services, or OPO operations
- Experience in federally regulated or oversight environments
- Track record launching new programs, workflows, or operational models
- Experience leading multidisciplinary clinical teams
- Strong documentation, decision-making, and escalation skills
- Ability to manage high-volume workloads with competing priorities
- Capacity to operate effectively with limited resources and tight timelines
- Advanced degree such as MBA, MHA, or related field
- Experience in large-scale clinical operations involving over 200 FTEs or multi-site systems
Work Environment
This is a hybrid role with up to 10% travel required. The position operates within a regulated healthcare environment, requiring strict adherence to privacy and security protocols. The investigator will work closely with clinical, operational, and technical teams to ensure safety systems are effective, scalable, and responsive.
