About the Role
This position involves leading the management of complex cases for dually eligible members by assessing health needs, developing care plans, and coordinating services across providers to improve outcomes and ensure continuity of care.
Responsibilities
- Evaluate health status and psychosocial needs of high-risk patients
- Develop individualized care management plans based on clinical assessments
- Coordinate transitions of care across healthcare settings
- Collaborate with physicians, social workers, and community resources
- Monitor patient progress and adjust care strategies as needed
- Ensure compliance with regulatory and accreditation standards
- Conduct regular follow-ups with members and caregivers
- Facilitate access to medical, behavioral, and social services
- Document care activities in electronic health record systems
- Participate in quality improvement initiatives
- Serve as a clinical resource for care teams
- Promote adherence to evidence-based treatment guidelines
- Support members in self-managing chronic conditions
- Respond to urgent care-related inquiries
- Lead interdisciplinary team meetings for complex cases
- Identify gaps in care and implement interventions
- Educate members on preventive health practices
- Maintain up-to-date knowledge of D-SNP program requirements
- Assist in member outreach and engagement efforts
- Ensure timely completion of required health assessments

Work Schedule
