Position Overview
The RN Hospitalist/Care Mgmt Liaison plays a key role in aligning clinical care with efficient discharge planning by serving as a communication link between Hospitalist providers and Care Management teams. This position supports patient-centered outcomes through consistent coordination, proactive planning, and timely resolution of care barriers.
Key Responsibilities
- Facilitate daily communication between Hospitalist physicians and Care Management staff to review and adjust patient discharge plans
- Attend interdisciplinary care meetings to contribute insights on patient progress and care transitions
- Act as a point of escalation for complex or high-risk cases, ensuring appropriate provider engagement
- Support updates to care plans based on physician direction and patient needs
- Promote collaboration across teams to enhance care efficiency and patient safety
Qualifications
Candidates must hold a Bachelor of Science in Nursing and be a licensed Registered Nurse with at least three years of clinical experience. Certification in Basic Life Support (BLS) is required. The ideal candidate thrives in fast-moving environments, demonstrates strong critical thinking, and communicates effectively with diverse stakeholders.
Prior experience in acute care, case management, utilization review, or discharge planning is preferred. Strong interpersonal skills and the ability to anticipate care needs are essential for success in this role.
