Sailor Health is looking for a compassionate and action-oriented RN Healthcare Advocate to join us in our mission to redefine the golden years. You will serve as a trusted partner for older adults on Medicare, offering empathetic guidance and tackling complex care challenges to ensure patients feel supported and empowered.
What You'll Do
- Build trusted relationships with older adults and their families, grounded in listening and empathy.
- Serve as a go-to resource for navigating medical systems, social services, and emotional wellness needs.
- Create individualized care plans that address both clinical needs and Social Determinants of Health.
- Address Social Determinants of Health such as access to food and housing programs, transportation coordination, medication management and reminders, appointment scheduling and follow-ups, and caregiver support and education.
- Collaborate with Sailor’s therapists, care coordinators, and tech platform to deliver seamless, person-centered care.
- Provide feedback to help shape Sailor’s patient advocacy model as we scale.
What We're Looking For
- RN license, active and in good standing.
- Prior Patient Advocacy Telehealth Experience.
- Based in the U.S.
Nice to Have
- Experience working with Medicare populations, ideally older adults.
- Familiarity with SDOH and how they affect patient outcomes.
- Clinical judgment balanced with empathy and problem-solving.
- Organized, communicative, and able to manage multiple patients and needs.
- Tech-savvy—comfortable learning new systems and working remotely.
- Action-oriented and energized by building something new.
