What You'll Do
In this hybrid position, you will serve as a clinical leader focused on improving health outcomes for individuals with kidney-related conditions. You'll work directly with healthcare providers across Central to Southern Kentucky, building trusted relationships and supporting practice teams in identifying opportunities for clinical improvement.
You will analyze performance data to guide provider education, focusing on closing care gaps, managing chronic conditions, and meeting quality benchmarks. A key part of your role includes sharing best practices, supporting workflow enhancements, and connecting eligible patients with care navigation services.
Collaboration with internal clinical teams ensures coordinated, high-quality care. You will also maintain accurate records of all engagements, aligned with national quality standards, and contribute to the development and rollout of targeted quality initiatives.
Requirements
- Active, unrestricted Registered Nurse license in the state of practice
- Bachelor’s degree in nursing or a related healthcare field
- Basic Life Support (BLS) certification obtained within 30 days of hire
- Minimum of 3 years of experience in healthcare, managed care, clinical operations, or care management
- Proven experience in quality improvement, clinical education, or provider engagement
- Valid driver’s license and auto insurance with regular travel required across assigned regions
- Residency within the state where the role is based
- Compliance with health screening, immunization, and wellness program requirements
Preferred Qualifications
- Experience with claims data or care gap closure initiatives
- Background in developing and maintaining provider relationships
Technical Skills
Proficiency in Microsoft Office applications including Outlook, Word, Excel, and PowerPoint is essential for reporting, communication, and documentation.
Work Mode
This role operates in a hybrid format with frequent travel required across Central to Southern Kentucky, particularly in Jefferson, Hardin, Larue, Bullitt, Nelson, and Marion counties. Flexibility in scheduling and site visits is expected.
