What You'll Do
Work directly with healthcare providers, physician offices, and patients enrolled in a specialized Kidney Management Program to support coordinated care and improved clinical outcomes. Serve as a trusted point of contact between care teams, patients, and provider networks, ensuring consistent engagement and program effectiveness.
Identify and develop new partnerships with providers to expand program reach and enhance service delivery. Use clinical data to pinpoint opportunities for improving patient care, focusing on chronic kidney disease management, care gap resolution, and performance metrics such as HEDIS and STAR.
Educate provider staff on best practices in kidney health, renal replacement therapies, and care workflows. Support patients by providing personalized education on kidney disease, co-morbid conditions, and treatment options, while connecting them with community resources and care support systems.
Advocate for patients by coordinating care across providers and support networks, helping individuals meet their health goals. Maintain accurate records of all interactions in compliance with NCQA standards and ensure all reporting is completed on time for internal and external stakeholders.
Requirements
- Hold a Bachelor’s degree and possess an active, unrestricted RN license
- Obtain Basic Life Support (BLS) certification within 30 days of hire
- Have a valid, unrestricted driver’s license and be prepared for frequent regional travel
- Reside in one of the designated South Carolina counties served by the program
- Comply with health and immunization policies, including TB screening, required vaccinations (e.g., Hepatitis B, MMR, Varicella, COVID-19, Influenza), and annual wellness assessments
- Follow all organizational procedures, HIPAA regulations, and federal, state, and local compliance standards
Preferred Qualifications
- At least three years of experience in clinical operations, care management, or quality improvement
- Background in care coordination or gap closure in outpatient or hospital settings
- Proven success in building and maintaining relationships with healthcare providers or patients
Technical Stack
Proficiency in Microsoft Office applications including Outlook, Word, Excel, and PowerPoint is required for daily operations, reporting, and communication.
Work Mode
This is a hybrid position requiring regular field visits. Employees must be able to travel frequently—more than 50% of the time—across designated counties in South Carolina, including Marlboro, Dillon, Marion, Horry, Georgetown, Williamsburg, Florence, Clarendon, Sumter, Lee, and Darlington.
