What You'll Do
Guide Peritoneal Dialysis (PD) patients through their treatment journey by delivering personalized education and clinical support. Train patients and caregivers in PD techniques and self-management to promote confidence and safety at home. Manage a defined caseload, conducting regular assessments, updating care plans, and monitoring progress toward health goals.
Perform in-home visits to evaluate patient environments and ensure proper adherence to treatment protocols. Provide on-call clinical support as needed and collaborate with interdisciplinary teams to coordinate care transitions and prevent hospitalizations. Help patients achieve greater independence while maintaining high standards of clinical quality.
Requirements
- Active Registered Nurse (RN) license in the state of practice
- Associate Degree in Nursing (ADN) required; BSN preferred
- Valid driver’s license, personal transportation, and current insurance
- Proficiency with basic computer applications, including MS Word and Outlook
Preferred Experience
- Experience in dialysis, including PD, home hemodialysis (HHD), or acute/chronic settings
- Background in patient education, case management, or telehealth
- Certification in Nephrology (CNN) or Dialysis Nurse (CDN) a plus

This is an onsite role based in Ithaca, New York, with structured flexibility to manage your caseload efficiently. Scheduling accommodates work-life balance, with weekends and holidays typically free. Reliable transportation is required for home visits.
