What You'll Do
In this remote position, you’ll deliver comprehensive case management to adult patients, their families, and caregivers across Virginia. You’ll assess individual needs, create care plans, and coordinate services that address both medical and psychosocial challenges, particularly during serious or terminal health conditions. You’ll work closely with interdisciplinary teams to ensure seamless delivery of care and advocate for resources that support patient well-being.
Your role includes monitoring ongoing care, adjusting plans as needed, and ensuring continuity across services. You’ll serve as a consistent point of contact for patients navigating complex health and social environments, helping them access appropriate support systems.
Requirements
- Hold a degree in Health & Human Services or a related field such as Social Work, Psychology, Nursing, Gerontology, Counseling, or Rehabilitation from a U.S. Department of Education-accredited institution
- Have at least one year of professional experience in long-term care
- Be a resident of Virginia, as this remote role requires in-state location
Degrees earned outside the United States will be evaluated on a case-by-case basis.
Preferred Qualifications
- One year of experience in health plan operations
- Experience working with Medicaid populations
- At least one year in a mental health setting
