What You'll Do
Begin each day remotely by reviewing patient data and identifying those recently hospitalized or at elevated risk. You’ll manage a caseload exceeding 100 individuals with advanced chronic kidney disease or end-stage renal disease, ensuring timely interventions. Travel to dialysis centers and nephrology offices will be required to meet patients during treatments or scheduled visits.
Perform thorough Comprehensive Health Evaluations, assessing medical, emotional, and social factors through head-to-toe evaluations. Educate patients on managing conditions like diabetes and heart failure, support medication reconciliation, and coordinate follow-up plans to reduce readmissions. You’ll collaborate closely with Nurse Practitioners and Care Coordinators to align care strategies and improve long-term outcomes.
Documentation of care plans and risk adjustment data is a key responsibility, typically completed from a home office. Your efforts will directly support reducing hospital utilization and improving patient health trajectories.
Requirements
You must hold an Associate Degree in Nursing from an accredited program and maintain an active Registered Nurse license in your state of practice. A minimum of five years of clinical nursing experience is required, along with current CPR certification. You should have a valid driver’s license with a clean driving record and meet employer insurance standards.
Proficiency in basic computer applications including MS Word and Outlook is essential. You must be able to adapt to digital workflows and new electronic systems, particularly Capella, achieving functional competence within 90 days of hire. A reliable home internet connection with at least 1MB upload and download speeds is necessary to support remote work.
Preferred Qualifications
A Bachelor of Science in Nursing is preferred. Alternatively, a three-year diploma from an accredited nursing program may be accepted. Prior experience in renal nursing—specifically three or more years—is highly valued. Demonstrated ability in performance coaching, change leadership, and continuous quality improvement (CQI) methods strengthens candidacy. Certification as a Nephrology Nurse (CNN) or Case Manager (CCM) is a plus.

Work Mode
This role begins with remote work, allowing you to start your day from home. Field visits to dialysis centers and nephrology offices in the Las Vegas area are required as part of regular responsibilities. The position supports a hybrid model combining home-based documentation and in-person patient engagement.
Compensation
Annual salary ranges from $68,400 to $100,400. Additional rewards include benefits eligibility beginning the first month after, or concurrent with, one month of continuous employment through the DaVita Rewards package.
