This role supports individuals with advanced chronic kidney disease and end-stage renal disease by managing a caseload of over 100 patients. Each day begins remotely, reviewing patient updates and identifying those recently hospitalized or at elevated risk. The nurse travels regularly to dialysis centers and nephrology offices to conduct in-person visits, ensuring continuity of care during treatment sessions.
What You'll Do
Perform comprehensive health evaluations that assess medical, emotional, and social factors, guiding personalized care strategies. Educate patients on managing conditions like diabetes and heart failure, support medication reconciliation, and coordinate follow-up plans to reduce readmission risks. Most documentation, including risk adjustment records and care plans, is completed remotely using electronic systems.
Collaborate with nurse practitioners and care coordinators to align treatment goals and improve long-term health outcomes. Emphasis is placed on proactive intervention, patient empowerment, and sustained care management.
Requirements
- Associate Degree in Nursing from an accredited program
- Active Registered Nurse license in the state of practice
- Minimum of five years’ clinical nursing experience
- Current CPR certification
- Valid driver’s license with a clean driving record and insurability
- Proficiency in MS Word, Outlook, and electronic health record systems
- Ability to adapt to evolving digital workflows, including Capella software, with functional competence expected within 90 days
- Home office setup with reliable internet (minimum 1 Mbps upload and download speeds)
Preferred Qualifications
- Bachelor of Science in Nursing
- Three-year nursing diploma accepted in lieu of degree
- Three or more years in renal nursing
- Experience in clinical performance improvement, outcome measurement, or care management
- Familiarity with Continuous Quality Improvement (CQI) methodologies
- Background in healthcare coaching or change leadership
- Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) credential
