The University of Rochester is hiring an RN Care Manager to manage the clinical aspects of patient-centered care, including chronic condition management. You will work with an interdisciplinary medical team to provide patient care and help develop program goals and initiatives, guided by our commitment to Meliora—Ever Better.
What You'll Do
- Coordinate medical care for patients identified as high-risk through assessment or physician determination.
- Assume a coordination role at the point of service through targeted outreach and follow-up.
- Assist in developing and implementing care plans for medically complex patients.
- Identify barriers to a successful care management path.
- Identify patient groups with chronic diseases.
- Assist in developing and implementing population-based strategies to close gaps in medical care.
- Identify resources for patient self-management skills.
- Coordinate the use of clinical and ancillary resources within and outside the health system to achieve treatment goals.
- Participate in program development by assisting with planning short-range and long-range goals for chronic disease management.
- Assist with developing current evidence-based protocols, policies, workflows, and guidelines related to the medical home model.
- Participate in committees as assigned.
- Interact effectively with physicians, home care teams, patients, and their caregivers.
- Be accountable for patient triage.
- Keep abreast of organizational developments and practices through continuing education, professional organizations, and current literature.
What We're Looking For
- Bachelor's degree in Nursing or a health-related field.
- 3 years of relevant experience.
- Current RN - Registered Nurse state licensure and/or Compact State Licensure upon hire.
Nice to Have
- Ambulatory care experience in community health, care management, disease management, quality improvement, and/or management experience.

Benefits & Compensation
- Salary range: $76,398.00 - $99,320.00.
Work Mode
This is a remote position. Candidates should be located in New York, United States, including the Albany area.
