Responsibilities
- Coordinate patient appointments within the electronic health record system to efficiently use clinical staff time while meeting patient and caregiver availability
- Respond professionally and promptly to incoming phone calls, text messages, and voicemail inquiries
- Oversee the handling of incoming and outgoing fax communications
- Input data, scan documents, and print records as required to support clinical operations and ensure accurate, up-to-date EHR documentation
- Assist residents with identifying and enrolling in appropriate health plan insurance options
- Collaborate with registered nurses and advanced practice providers to coordinate care activities that improve patient results
- Partner with the local Savoy team to connect with community services that support patient care objectives
- Facilitate telehealth appointments using a tablet to connect residents with virtual care providers
- Conduct in-person visits with residents as part of ongoing care coordination responsibilities
- Maintain clear, consistent communication and build strong relationships with residents, caregivers, family members, healthcare providers, pharmacies, and community staff
- Promote engagement with eligible residents, power of attorney holders, and families by educating them on available services and securing signed consent forms
