CareHarmony is hiring a Remote Triage Nurse to provide exceptional patient care during after-hours and weekend shifts. In this role, you will conduct telephonic triage for complex patients, assist with psychosocial needs, and ensure high-quality care under the direction of the Lead Triage RN. Our team is driven by a passion for improving care delivery and helping patients navigate the healthcare system.
What You'll Do
- Conduct thorough telephonic patient assessments using proprietary tools
- Identify and address patient concerns and acute clinical issues
- Collaborate closely with patients’ providers to determine appropriate care
- Provide telephonic education and support to patients
- Participate in projects with the Innovation team
- Ensure excellent quality of care during all on-call and triage interactions
- Foster positive team relationships to support a collaborative culture
What We're Looking For
- Active Compact/Multistate Nursing Licensure (LPN/LVN or RN)
- Graduate of an accredited School of Nursing
- Active, good-standing nursing license
- A minimum of five (5) years of professional nursing experience
- Direct experience in telephonic triage
- Innovative mindset focused on improving patient health management
- Robust clinical knowledge and assessment skills
- Ability to quickly determine appropriate level of care
- Ability to navigate multiple technological platforms
- Excellent attention to detail, organizational, and customer service skills
- Excellent verbal and written communication skills
- Professional presence when dealing with colleagues, clients, and patients
- Ability to function cohesively within a team
- Must have active high-speed Wi-Fi
- Must have a home office or HIPAA-compliant workspace

Team & Environment
You will report directly to the Lead Triage RN and work as part of a remote team dedicated to consistent, high-quality patient care.
Benefits & Compensation
- Compensation: $25/hour for LPN/LVN or $27/hour for RN
