CareHarmony is hiring a Remote Triage Nurse to work Weekend Days as a Licensed Practical Nurse (LPN) or LVN. In this role, you will provide exceptional patient care for all after-hours calls under the direction of the Lead Triage RN. What You'll Do
- Conduct thorough telephonic patient assessments using proprietary tools.
- Identify, address, and triage patient concerns and acute clinical needs.
- Collaborate closely with patients' providers to address clinical concerns.
- Provide telephonic education to patients.
- Participate in Innovation team projects.
- Ensure excellent quality of care during all on-call and triage interactions.
- Foster positive team relationships to support a supportive CareHarmony culture.
What We're Looking For
- Compact/Multistate Nursing Licensure is required.
- Graduate of an accredited School of Nursing with an active, good-standing license.
- A minimum of five (5) years of professional nursing experience.
- Specific experience in Triage and extensive experience in telephonic triage.
- Robust clinical knowledge and ability to quickly determine the appropriate level of care.
- Ability to navigate multiple technological platforms.
- Excellent attention to detail, customer service, organizational, and communication skills.
- Excellent professional presence with colleagues, clients, and patients.
- Ability to function cohesively within a team.
- Must have active high-speed Wi-Fi and a home office or HIPAA-compliant workspace.

Team & Environment
You will report directly to the Lead Triage RN.
Benefits & Compensation
- Compensation is $25/hour for LPNs and $27/hour for RNs.
