Chronic Care Staffing is looking for a Remote RPM Coordinator (CMA/RMA) to provide essential support for patients managing chronic or high-risk conditions. You will deliver compassionate care through Remote Patient Monitoring (RPM) services, ensuring patients receive timely attention and guidance.
What You'll Do
- Conduct patient onboarding and provide clear device education for new RPM enrollees.
- Monitor daily biometric data streams, including blood pressure, glucose, weight, and oxygen saturation.
- Identify abnormal readings and follow established escalation protocols to notify healthcare providers.
- Perform monthly RPM engagement calls to assess patient condition and address any concerns.
- Troubleshoot device connectivity issues and help patients resolve usage problems.
- Communicate effectively with patients and caregivers regarding care goals and progress.
- Support patient self-management and adherence to individualized care plans.
- Coordinate with healthcare teams to ensure timely responses to clinical alerts.
- Document all patient interactions accurately in EHR and RPM platforms.
- Ensure compliance with CMS RPM billing and documentation guidelines.
- Meet productivity expectations for monitoring, outreach, and follow-up tasks.
- Represent Chronic Care Staffing with professionalism and empathy in all interactions.
What We're Looking For
- Active, unencumbered CMA or RMA certification.
- Current BLS certification.
- Strong ability to communicate clearly and compassionately via phone or messaging.
- Excellent documentation, time management, and critical thinking skills.
- Experience working with EHR systems and cloud-based healthcare platforms.
- Familiarity with Google Suite.
Nice to Have
- Strong understanding of RPM protocols, workflows, and monitoring platforms.
- Ability to recognize and respond appropriately to clinical red flags in biometric data.
