CircleLink Health is hiring a Remote Registered Nurse Care Coach to serve Medicare patients with multiple chronic conditions. In this part-time 1099 contractor role (20-25 hours/week), you will provide monthly telephonic coaching to improve health outcomes and reduce hospitalizations.
What You'll Do
- Utilize specialized care management software to call Medicare patients with 2+ chronic conditions monthly.
- Build rapport and coach patients to improved health using SMART goals and self-management education.
- Implement and update the Plan of Care by tracking medications, appointments, biometrics, and symptoms.
- Connect patients with community resources for transportation, personal care, prescription assistance, and social services.
- Conduct Transitional Care Management activities for high-risk patients post-discharge to reduce readmissions.
- Close preventive care gaps by encouraging annual well visits, vaccines, cancer screens, and specialist appointments.

- Good with technology and eager to learn and use new software.
- Excellent organizational and time management skills.
- Strong communication and telephonic skills.
- Strong critical thinking and problem-solving skills.
- Current, unrestricted Compact License or an RN license in: Georgia, Indiana, North Carolina, or Maryland, USA.
- Proficiency with electronic health records and web-based applications.
- 3+ years' experience as a Registered Nurse.
- STRONG internet-connected computer.
- Minimum of 20 hours of availability per week.
Nice to Have
- Spanish fluency.
- Case Management or Chronic Disease Management experience highly preferred.
- Certified Diabetes Educator.
- Experience with Motivational Interviewing or other behavior change communication techniques.
