DaVita is hiring a Remote Licensed Social Worker to provide vital psychosocial support to dialysis patients as a core member of their care team. In this role, you'll build meaningful long-term relationships and help patients manage the multifaceted demands of End Stage Renal Disease. The position serves a caseload across multiple clinics and requires multi-state licensure.
What You'll Do
- Provide psychosocial health and wellbeing interventions for dialysis patients.
- Complete mandatory assessments, screenings, and education for patients.
- Build meaningful and long-term relationships with patients and their families.
- Support and advocate for patients adjusting to life on dialysis.
- Help patients understand their rights and responsibilities.
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
What We're Looking For
- Master's degree in Social Work (MSW) with a specialization in clinical practice.
- LSW licensure.
- Meets all state required regulations to practice in the dialysis setting.
- Willingness to obtain multi-state licenses (cost will be reimbursed).
- Demonstrated knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
- Functional proficiency with DaVita specific applications within 60 days.
