H!KINEX is hiring a Remote Licensed Practical Nurse (LPN) with an active Oregon license. In this Care Coordinator role, you will work comprehensively with providers to deliver value-based care management initiatives and coordinate resources for patients.
What You'll Do
- Deliver value-based care management initiatives for patients in collaboration with providers.
- Identify resources and coordinate needs for chronic care management patients.
What We're Looking For
- Active Licensed Practical Nurse (LPN) licensure in the state of Oregon.
- At least 3 years of direct patient-facing work experience.
