Responsibilities
- Monitor patient conditions using remote tools and documentation systems
- Conduct virtual check-ins with patients to assess health status
- Collaborate with healthcare providers to adjust care plans
- Document patient interactions in compliance with regulatory standards
- Provide education on medication management and treatment plans
- Respond to patient inquiries within established response timeframes
- Support care transitions between facilities and home settings
- Maintain accurate electronic health records
- Follow protocols for reporting changes in patient condition
- Participate in team huddles and care coordination meetings
- Ensure adherence to infection control and safety guidelines
- Assist in developing individualized care goals
- Use telehealth platforms to deliver services
- Report urgent clinical issues to supervising clinicians
- Uphold patient confidentiality and HIPAA requirements
- Engage in ongoing training and professional development
- Follow clinical workflows and organizational policies
- Assist with remote monitoring device setup and troubleshooting
